Although the symptoms are similar, a few key differences will help you identify whether the injury affected the ACL or MCL. An ACL tear will have a more distinctive and loud popping sound than an MCL tear. Additionally, the location of your pain and swelling could indicate either an ACL or MCL tear.

It can be easy to confuse the two injuries, as both a torn ACL and a torn MCL exhibits similar symptoms, including: swelling, inflammation, extreme pain and possible bruising. The main difference between an ACL tear and an MCL tear is that an ACL tear will have a distinctive popping sound, while an MCL tear will not.

After management of acute ACL rupture with a novel bracing protocol, 90% of patients had evidence of ACL healing on a 3-month MRI (continuity of the ACL). More ACL healing on a 3-month MRI was associated with better knee function and QOL, less passive knee laxity and a higher return-to-sport rate.
Occasionally, theyll have been misdiagnosed with a knee sprain when the issue is far more serious. Unfortunately, ACL injuries are rather good at disguising their true nature, and that can present real problems.
