Today were going to take a look at creating data entry forms in Excel. Theres going to be no VBA, its going to be super easy. (groovy beat music) So lets say for one reason or another, you have to input some data manually in Excel. And you want to make the experience as pleasant as possible. You can use data forms to do that. Its especially good if your tables are really wide and you want to avoid horizontal scrolling. Now, just to keep things simple, in this example, I have a small table where I want to input data in category, task, date, person, and status. One way of entering data is just to input it here, right? Ill put in site, task, lets say health check, date, person, James. Now if I wanted to edit something, I have to go back to that cell and then click, go to edit mode and then expand that task. An alternate way of doing this is to use a form. A DNR order only applies when the heart or breathing has stopped. It does not affect care before the heart has stopped, for pain, shortness of breath or other symptoms.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
DNR protocols typically require the signing of an out-of-hospital DNR order by both the clinician and patient (or the patients surrogate) and the use of a special identifier (eg, a bracelet or brightly colored form) that is worn by or kept near the patient.
Do-Not-Resuscitate (DNR) A DNR order indicates to your doctors and other healthcare providers that you dont want CPR attempted in the event your heart stops beating or your breathing stops. In most cases, your doctor writes a DNR order after discussing the burdens and benefits of CPR with you and your family.
Regardless of the format or the venue, DNR orders almost always follow the same general rules to be valid: DNR orders must be written by medical professionals rather than verbalized. Written DNR orders must include the patients name. DNR orders must be dated. The DNR order must be signed by the healthcare provider.
The most accurate form of identification for patients outside of licensed facilities is a medallion or bracelet attached to the patient. Use of such a medallion should never make the patient uncomfortable and should always be optional. Every local EMS system should accept an EMSA approved DNR bracelet or medallion.
Question: What happens if you resuscitate someone with a DNR? If you attempt resuscitation on a person who has a valid DNR order, you open yourself up to lawsuit by the estate (read: family) of the person with the DNR order, especially if the attempt is successful and they are indeed brought back to life.
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes. Second, the form must be signed off by the attending physician.
