Even if the server doesnt have to cover that credit card service fee, your cash tip will help him or her out by giving the money immediately (rather than the server having to wait until their paycheck for the tip). The good folks at Chow suggest writing Cash on the tip line of your receipt to avoid confusion.
You dont have to, but as Gary pointed out - youre leaving yourself open for abuse by the person receiving the slip. If you sign it and leave the tip and total blank, youre effectively giving the person the opportunity to fill in their own tip.
The consensus is that you should probably be tipping on the total, but there are arguments in favor of tipping on the subtotal as well.
At your next meal out, do NOT leave a blank copy of your receipt on the table when you leave. While 99.99% of servers would never dream of adding a generous tip to your unsigned, abandoned receipt (thats what youre there for!), there is the opportunity for this to happen if you leave a blank copy at the restaurant.

The charge should be processed based on the total you wrote on the receipt, regardless if your math is accurate. I have seen situations where the total has been corrected and generally if the customer does not complain to his or her credit card issuer, then the charge will go through for the revised amount.
The IRS requires that all meal expenditures must document the following information (regardless of cost): List of attendees (self if alone) The business purpose of the meal (topic of discussion) The cost (supported by receipt)
