Keeping the trigger guard free of obstruction when holstering prevents discharges of that type. Keeping your finger off the trigger unless you intend to shoot keeps them from happening. And carrying a pistol in a holster that fully and adequately protects the trigger guard does as well.

There is no right or wrong answer when it comes to carrying with a manual safety. Ultimately, the decision comes down to your personal preference and comfort level.
Most striker-fired and double-action only firearms can reasonably be carried without the safety on, again assuming your safety and holstering skills are in place. Single action pistols like the Springfield 911 MUST be carried with safety on.

Ultimately, practicing proper safety techniques and utilizing a secure holster can make carrying without a manual safety just as safe as carrying with a manual safety.
