First reported by Dutch Professor of Cardiology, Robbert J. de Winter in 2008, the de Winter ECG pattern is an anterior STEMI equivalent that presents without obvious ST segment elevation. These patients are suffering occlusion myocardial infarction (OMI) and require immediate reperfusion therapy. Wellens Syndrome requires urgent coronary intervention to prevent a myocardial infarction, while Brugada Syndrome necessitates strategies to mitigate the risk of sudden cardiac death, including possible ICD placement. Two EKG Patterns You Must Know Before Your EM Rotation: Wellens EMRA students wellens-and-brugada EMRA students wellens-and-brugada
Inferior STEMI In general, inferior STEMIs result from the occlusion of the Right Coronary Artery (70%) or the Left Circumflex. On ECG, this can be recognized with ST elevations in the inferior leads (II, III and AVF) with reciprocal changes in the high lateral leads (I and aVL).
An acute ST-elevation myocardial infarction (STEMI) is an event in which transmural myocardial ischemia results in myocardial injury or necrosis. [1] The current 2018 clinical definition of myocardial infarction (MI) requires the confirmation of the myocardial ischemic injury with abnormal cardiac biomarkers.
The de Winter electrocardiogram pattern is a finding that, in a proper setting, is highly suggestive of acute occlusion of the left anterior descending artery. The de Winter ECG Pattern - StatPearls - NCBI Bookshelf National Institutes of Health (NIH) (.gov) books NBK557573 National Institutes of Health (NIH) (.gov) books NBK557573
A Wellens syndrome type B pattern with deep and symmetrical negative T waves from V2 to V5 is observed. De Winters pattern refers to a negative slope of the ST segment from V1 to V3, with a tall and pointed T wave in addition to an elevation of the ST segment in aVR.
When theres an elevation in the ST segment, that often means theres a total blockage of one of the hearts main supply arteries. When that is happening during a heart attack, it can be a sign that the muscle of the ventricles is dying.
Although the Wellens pattern is highly specific for stenosis of the LAD coronary artery by an occlusive plaque, there exist mimics of Wellens syndrome or so-called pseudo-Wellens syndrome. Causes of this include cocaine use, in which coronary vasospasm can result in the typical Wellens ECG pattern.
