The index of microcirculatory resistance (IMR) is a pressure-temperature sensor guidewire-based measurement, performed during cardiac catheterization, of the minimum microcirculatory resistance in a target coronary artery territory (7).
In patients with both acute coronary syndromes and stable coronary disease, oxygen administration may constrict the coronary vessels, lower myocardial oxygen delivery, and may actually worsen ischemia.
Acute Myocardial Infarction (AMI), commonly known as a heart attack, occurs when there is a sudden blockage of blood flow to a portion of the heart muscle. AMI is a medical emergency that requires immediate medical intervention to restore blood flow and prevent further damage to the heart.
The microvascular resistance reserve (MRR) has recently been introduced as a novel index to assess the vasodilatory capacity of the microcirculation, independent of epicardial disease. The prognostic value of MRR in ST-segment elevation myocardial infarction (STEMI) is unknown.
Acute myocardial infarction is myocardial necrosis resulting from acute obstruction of a coronary artery. Symptoms include chest discomfort with or without dyspnea, nausea, and/or diaphoresis. Diagnosis is by electrocardiography (ECG) and the presence or absence of biomarkers.

However, oxygen therapy fails to result in better outcomes. The Air Versus Oxygen In MI (AVOID) study showed that supplemental oxygen in STEMI patients may increase early myocardial injury rates and is associated with a larger MI size.
