Chronic heart failure (HF) is associated with mild to moderate changes in pulmonary function, including restrictive and obstructive changes as well as a reduction in lung diffusing capacity (DLCO) 1-3.
The physical examination should include Valsalva's maneuver, a test that is highly specific and sensitive for the detection of left ventricular systolic and diastolic dysfunction in patients with heart failure. An electrocardiograph and a chest radiograph should also be obtained.
While both types can cause shortness of breath, obstructive lung diseases, such as asthma and chronic obstructive pulmonary disorder (COPD), cause more difficulty with exhaling air, while restrictive lung diseases (such as pulmonary fibrosis) can cause problems by restricting a person's ability to inhale air.

While most patients with suspected HF do not require invasive testing for diagnosis, the clinical gold standard for diagnosis of HF is identification of an elevated pulmonary capillary wedge pressure at rest or exercise on an invasive hemodynamic exercise test in a patient with symptoms of HF.
Severe heart failure patients often show a restrictive respiratory pattern, secondary to heart enlargement and increased lung fluids, and impairment of alveolar-capillary gas diffusion, mainly due to an increased resistance to molecular diffusion across the alveolar capillary membrane.
Major cardiac effects caused by beta blockade include the precipitation or worsening of congestive heart failure, and significant negative chronotropy.
Beta blockers should not be administered to patients with heart failure who have bradycardia, heart block or hemodynamic instability. Patients hospitalized for heart failure may receive beta blockers only after they have been stabilized.
The use of β-blockers (BB) in heart failure (HF) has been considered a contradiction for many years. Considering HF simply as a state of inadequate systolic function, BB were contraindicated because of their negative effects on myocardial contractility.
