A checkbox group can include a validation rule to enforce how many checkboxes in the group your signer must select. Checkboxes can be required or optional, but if you apply validation that requires the signer to select a certain number of boxes, then the field becomes required.
For Windows and Mac PC users In the Word Options window, select Customize Ribbon. On the right side, under Main Tabs, find and check the Developer Box. In this group, click on the Checkbox Content Control. Choose Symbol, select a font like Wingdings, and thenselect a checkbox-like symbol.
Click File Options Customize Ribbon. Then, check the box next to Developer and click OK. In the Controls group of the Developer tab, click on the Check Box Content Control button. This inserts a checkbox at the cursors location.

Change the check box symbol In your document, click the check box. On the Developer tab, click Properties. In the Content Control Properties dialog box, selectChange next to Checked symbol. In the Symbol box, look for a symbol.
To add a content control to a document in Word On the Ribbon, click the Developer tab. If the Developer tab is not visible, you must first show it. For more information, see How to: Show the Developer tab on the Ribbon. In the Controls group, click the icon for the content control that you want to add.
Position your cursor where you want to place the checkbox in the document. Get back to the Developer tab and click on the Check Box Content Control button. You will see a checkbox appear wherever your cursor is placed. To mark a checkbox, click on it and add an X.
