The affidavit must be made in the presence of an authorized affidavit taker, and it’s important to verify the format requirements for different courts or tribunals beforehand. Most of the affidavit form can be completed prior to the meeting, but it should not be signed until in the presence of the affidavit taker. After signing or initialing any changes, both the individual and the affidavit taker must sign each page. If the affidavit references any document, the affidavit taker must attach a certificate to identify it. A list of authorized affidavit takers is available at justice.vic.gov.au.
