All services provided on the day of discharge from inpatient status are coded 99238 or 99239. This applies for a discharge from inpatient status on a day other than the day a patient was admitted.
The Hospital Discharge Day Management Service (CPT code 99238 or 99239) is a face-to-face evaluation and management (E/M) service with the patient and his/her attending physician.
CPT Code 99239 - Hospital Inpatient or Observation Discharge Services - Codify by AAPC.
A patient discharge status code is a two-digit code that identifies where the patient is at the conclusion of a health care facility encounter or at the end of a billing cycle (the through date of a claim).
There are two CPT codes to choose from for these services 99238 and 99239 and the difference between them comes down to time. If the entire discharge, including all preparation, takes 30 minutes or less, you need to report 99238. If, on the other hand, the process takes more than 30 minutes, you should report 99239.
Billing for a 99239 requires documentation of the total time spent by the practitioner discharging the patient and the time must be 30 minutes.) Time spent by a resident or team members other than the practitioner cannot be utilized to account for the total time to support a 99239.
Answer: CPT 99238 describes a discharge service of up to 30 minutes duration; it does not include a minimum time requirement, so documentation of time is not required. The related higher level code, CPT 99239, has a minimum time requirement of 30 minutes or more, so time notation is a requirement for 99239.
