The majority of patients who have partial ACL tears can recover without surgical intervention. Recovery can take 6-9 months. However, rushing the healing process increases the risk of making a small tear worse. A knee brace and/or crutches help to keep your knee stable through the early stages of recovery.
Without surgery, however, there is an increased risk of developing knee osteoarthritis. An HSS study of 40 surgical and nonsurgical patients over more than 10 years found that the long-term effects of an ACL tear without surgery includes damage to the surrounding cartilage, which worsens over time.
Very minor tears (sprains) may heal with non-surgical treatments and regenerative medicine therapy. But full ACL tears cannot be healed without surgery. If your activities do not involve making pivoting movements on the knee, physical therapy rehabilitation may be all you need.
Arthroscopic debridement is a surgical procedure that removes the broken down bits of cartilage and tissues to help reduce pain and improve movement. It is most commonly performed to help reduce the symptoms of arthritis so that you can regain much of the function of your knee while reducing pain.
If nothing is done, the ACL injury may turn into chronic ACL deficiency. Your knee may become more and more unstable and may give out more often. The abnormal sliding within the knee also can hurt cartilage. It can trap and damage the menisci in the knee and can also lead to early osteoarthritis.
In general, the recovery time for ACL tears is about six to nine months without surgery. However, balancing resting the ACL with therapeutic movement can help keep you in the lower end of this range. Rest is important for ACL tears because your body does most of its healing when you arent moving.
