Clean Claims are claims that have all information in them and nothing is missing. If any mandatory or conditional information is missing, the claim will be considered unclean. Examples of unclean claims include invalid member ID, provider data discrepancy NPI and atax ID does not match.
A claim rejection occurs before the claim is processed and most often results from incorrect data. Conversely, a claim denial applies to a claim that has been processed and found to be unpayable. This may be due to terms of the patient-payer contract or for other reasons that emerge during processing.
1. Clean claim defined: A clean claim has no defect, impropriety or special circumstance, including incomplete documentation that delays timely payment.
The clean date is the date when the requested information required for resolving the suspended line(s) is received, and the claims processing system of record is updated by removing the non-clean EX code to populate the clean date.
Incomplete Claim means a claim which, if properly corrected to completion, may be compensable for the covered procedure, but lacks important or material elements which prevent payment of the claim. Incomplete Claims shall be denied if not cured within 30 days of notice of the lack of completeness.

A clean claim means a claim that does all of the following: Identifies the health professional, health facility, home health care provider, or durable medical equipment provider that provided service sufficiently to verify, if necessary, affiliation status and includes any identifying numbers.
Claim: This is defined as a formal request for your insurance company to provide coverage for your medical expenses. EOB: A document that shows how much the insurance paid, your responsibility and what information may be needed to complete your claim.
(ii) Clean claim defined In this paragraph, the term clean claim means a claim that has no defect or impropriety (including any lack of any required substantiating documentation) or particular circumstance requiring special treatment that prevents timely payment from being made on the claim under this part.
