The ACL has a microstructure of collagen bundles of multiple types (mostly type I) and a matrix made of a network of proteins, glycoproteins, elastic systems, and glycosaminoglycans with multiple functional interactions.
MRI Anatomy of the Cruciate Ligaments. On sagittal T2-weighted images, the normal ACL appears as a straight band parallel to the intercondylar roof (Figure 1A). It fans out as it descends to its broad-based anterior tibial attachment.
The average length is 33 mm (1.3 inches) and the width is 11 mm ( 0.43 inches). The anatomy and footprint of the ligament are very important in reconstruction. The primary function of the ACL is to limit anterior translation (forward sliding) of the tibia on the femur in the knee joint.
Complete ACL tears cannot heal on their own, and therefore, almost always require surgical intervention. The ligament, given its location in the knee, cannot spontaneously heal because it has poor blood supply to it.
The midsubstance of the ligament has a rounded or elliptical cross-section, with an oval isthmus and larger insertion areas, so the ACL also has been described as having the shape of an hourglass55 or a bow tie56.
Injured ligaments on MRI may appear disrupted, thickened, heterogeneous, or at tenuated in signal intensity, and may be ab normal in contour. Fluidsensitive sequences are often helpful in detecting injury.
The results of this study showed that the sensitivity of MRI in diagnosing ACL damage was 95.45%, and the specificity was 91.67%. Additionally, this study used MRI to diagnose ACL tears in patients with ACL injuries.

In general, Grade 1 ACL tears are treated with methods such as bracing, nonsteroidal anti-inflammatories (NSAIDs) for pain, and RICE (rest, ice, compression, and elevation). When these precautions are taken, general healing time is about 2-4 weeks.
The articular cartilage provides lubrication and as a result, there is very little friction when the joint moves. This joint cartilage can get damaged when the ACL tears and the joint is compressed in an abnormal way. If this articular cartilage is injured, the joint no longer moves smoothly.
