I was looking for a way to explain our medical records quickly to new doctors I developed a page with six months and under each month was the day of the month when we saw the doctor or add the test at the end of six months I had a sheet that looked like this and I decided to condense it even further with only the major tests and things other doctors would want to know so my original sheet looked like this with every single appointment then my condensed sheet had just the major things other doctors would want to know then I took that six months and put it together with the next three years of appointments and created two pages that showed three and a half years we would go into the doctor's office and makanda would open this up and answer the new doctor's questions that new doctor would look so interested they'd want to take it so I'd pull another sheet out and say here's your copy to take notes the doctors were very appreciative of the quick summary this way they could look and see wh...
