You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.
Keep first aid records Date, time and place of the incident. Name of the person affected. Details of the injury or illness, and what first aid was given.
Within seven calendar days after you receive information that a recordable work‐related injury or illness has occurred, you must fill out this form or an equivalent.
Both Federal employees and contractors may complete an OSHA 301 when a recordable injury or illness has occurred.
What information should be recorded? The date, time and place of the incident. The name and job of the injured or ill person. Details of the injury/illness and what first aid was given. Details about what happened to the person immediately afterwards (e.g. went back to work, went home, went to hospital).
Establishments with 100 or more employees in the highest-hazard industries to submit Form 300 Log and Form 301 Incident Report information once a year to OSHA. These establishments would continue to be required to electronically submit information from their Form 300A Annual Summary.
OSHA Form 301 is used by employers to create a detailed record of workplace injuries and illnesses. Employers must save the annual summary of all reports filed. Employers must also save the OSHA 301 Incident Report forms for 5 years after the end of the calendar year that the records cover.
First aid can include cleaning minor cuts, scrapes, or scratches; treating a minor burn; applying bandages and dressings; the use of non-prescription medicine; draining blisters; removing debris from the eyes; massage; and drinking fluids to relieve heat stress.
