Cervical cancer is cancer of the cervix, the lower part of the uterus that opens into the birth canal. It is one of the most common types of cancer in women worldwide, but also one of the most preventable, thanks to early detection with Pap tests. The cervix has 2 major cell types: flat squamous cells lining the outer part, and column-shaped glandular cells covering the inside of the cervical canal. Both types can become cancerous but squamous cell carcinomas are much more common. Cancer usually starts in the zone where the two cell types meet, known as the transformation zone. Virtually all cervical cancers are caused by human papillomaviruses, or HPVs. There are over a hundred different types of HPV, some of which pose higher risks than others. About 70% of all cases are caused by just two types: HPV-16 and HPV-18. Two proteins produced by HPV, known as E6 and E7, interfere with cell functions that normally prevent excessive cell division. The Pap test does not check for ovarian cancer. The only cancer the Pap test screens for is cervical cancer. Since there is no simple and reliable way to screen for any gynecologic cancer except for cervical cancer, it is especially important to recognize warning signs, and learn what you can do to reduce your risk.
Pap smear results indicate whether cervical cells appear normal, abnormal, or if the test result is unclear. Abnormal Pap smear results are described ing to the type of abnormality detected, as well as the type of cell in which the abnormality was found.
Heres what youll encounter during your first exam: You will fully undress and get into a hospital gown. A breast exam will be first. Youll lie down on the exam table, where your provider will perform a standard breast exam.
Do not use douche, bubble bath, or use vaginal medicine for three days before Pap test. You may shower, but do not tub bathe 24 hours before the Pap test. Inform your clinician about additional medications/conditions that may interfere with an accurate test.
A normal Pap smear shows healthy squamous cells (flat cells that look like fish scales) from the surface of the cervix. There are no signs of infection and no abnormal cells. Even if your Pap results are healthy, you should be tested regularly.
It doesnt take much to prepare for a pap smear. Some women may feel like they need to shave their pubic hair, but its unnecessary for this test. You should only tackle it if youll be more comfortable. Your doctor has seen it all, so a little bit of pubic hair wont bother him.
While these studies suggest that a large proportion (49%72%) of patients with cervical cancer had either not been screened or had been improperly screened, as many as 30%50% of women with cervical cancer had had a normal Pap result in the 35 years before their cervical cancer diagnosis.
How you prepare Avoid intercourse, douching, or using any vaginal medicines or spermicidal foams, creams or jellies for two days before having a Pap smear, as these may wash away or obscure abnormal cells. Try not to schedule a Pap smear during your menstrual period.
Results from your HPV test will come back as either positive or negative. Positive HPV test. A positive test result means that you have a type of high-risk HPV thats linked to cervical cancer. Negative HPV test. A negative test result means that you dont have any of the types of HPV that cause cervical cancer.
Preparations Before the Test Make sure not to schedule your Pap smear test during your period. Wear something comfortable and easy to remove. Your doctor may ask to remove clothes from the waist down, but others ask their patient to remove all clothes, especially when they suggest a breast exam.
