Multiple Procedures (Modifier 51) The multiple procedures modifier identifies the secondary, additional or lesser procedures for multiple procedures that are performed on the same day or at the same operative session.
Most of the procedures performed in ASCs have a global period of 10 or 90 days, but that global period applies to the operating physician's claims — not the facility's claims. Therefore, ASC coders will rarely need to use modifiers 78 or 79.
Modifier 58 can still be applied when the subsequent procedure is unplanned, as long as it is related to the cause of the original procedure and is more extensive than the original. Be sure that no other modifier should apply, specifically modifier 78, since the two are easily confused.
An ASC uses a combination of physician and hospital or clinical billing, employing the CPT and HCPCS level codes (as do most physicians), some insurance carriers permit an ASC to bill using ICD-10 procedure codes as does a hospital.

Ambulatory Surgery Centers—known as ASCs—are modern health care facilities focused on providing same-day surgical care, including diagnostic and preventive procedures.
ASCs use modifier -74 for surgical procedures terminated after administration of anesthesia or initiation of the procedure. Contractors make full payment for the surgical procedure if a medical complication arises causing the procedure to be terminated after anesthesia has been induced or the procedure initiated.
Choosing the correct billing form Hospital-based ASCs use the UB-04 form, while freestanding ASCs generally use the CMS 1500 form.
Coding for Ambulatory Surgery Centers is a specialty unto itself. It is a facility service, but Medicare requrires ASC's to send their bills to the professional fee (Part B) payers, but using the facility fee (Part A) claim form. There is a whole different set of regulations and bundling edits to use for ASCs.
