To change the name on your childs birth certificate, contact your states Office of Vital Records (typically part of the Department of Health). Many states allow new parents six to 12 months to make changes on a childs birth certificate without requiring a court order.
If it is your record, or the record of your child or spouse, you can contact the Illinois Department of Public Health (IDPH) to correct the error. Fill out the Affidavit and Certificate of Correction Request. Follow the instructions on the first page of the form. You must sign the form in front of a notary.
In order to apply for a change of name, an application form needs to be completed and posted to the register office where the entry is held. The birth certificates with the old name need to be given to the register office along with an application form and the relevant fee for a new certificate.
