The Attestation of Death is a form that is completed by the physician or nurse who attests the death. If the death cannot be attested by a physician or nurse within a reasonable timeframe and it is evident that the person has died, the Attestation of Death may be drawn up by two peace officers.
The Qubec civil status register contains information regarding births, marriages, civil unions and deaths that occurred in Qubec or pertain to person domiciled there. The register can only be consulted through an application for a certificate, a copy of an act or an attestation.

What is an act of death? An act of death is a document confirming someones death. In Quebec, a government agency called the Directeur de ltat civil (registrar of civil status) draws up the act of death. Like acts of birth, marriage, and civil union, acts of death are entered in a register kept by the Directeur.
