The hallmark of heart failure is dyspnea. The classic combination of raised jugular venous pressure (JVP), peripheral edema, palpable liver, basal crepitations, tachycardia, and a third heart sound is well known.
The sympathetic nervous system is activated in heart failure, via low and high pressure baroreceptors, as an early compensatory mechanism which provides inotropic support and maintains cardiac output.
Heart failure sometimes known as congestive heart failure occurs when the heart muscle doesnt pump blood as well as it should. When this happens, blood often backs up and fluid can build up in the lungs, causing shortness of breath.
Therefore, the systemic responses (vascular constriction and increased blood volume) help to compensate for the loss of cardiac performance; however, these compensatory responses cause a large increase in venous pressure that can lead to edema.
The sympathetic nervous system is activated in heart failure, via low and high pressure baroreceptors, as an early compensatory mechanism which provides inotropic support and maintains cardiac output.
Compensation in the body The bodys hormone and nervous systems try to make up for this. They increase blood pressure, hold on to salt (sodium) and water in the body, and increase the heart rate. These responses are the bodys attempt to compensate for the poor blood circulation and the backup of blood.
After this initial decline in pumping capacity of the heart, a variety of compensatory mechanisms are activated, including the adrenergic nervous system, the renin-angiotensin system, and the cytokine system.
Congestive heart failure is a syndrome that can be caused by a variety of abnormalities, including pressure and volume overload, loss of muscle, primary muscle disease or excessive peripheral demands such as high output failure. In the usual form of heart failure, the heart muscle has reduced contractility.
Vascular. Overall, the changes in cardiac function associated with heart failure result in a decrease in cardiac output. This results from a decline in stroke volume that is due to systolic dysfunction, diastolic dysfunction, or a combination of the two.
Both the 2021 ESC and 2022 ACCF/AHA/HFSA guidelines firmly establish that the four pillars of ARNI, evidence-based -blockers, MRA, and SGLT2i now form the foundational standard of medical therapy for patients with HFrEF (Figure 2). Each of the four therapies received the highest class of recommendation (Class I).
