Go to Document Header and Footer. Type your information into the header where you want it to show on the page. Note that this information will appear on every page of the script. If you need custom information for a particular section of the script (i.e. act or scene number) you should use a label.
The actual title should be written in ALL capital letters. This can also be bold or underlined, but no matter what, it must ALWAYS be capitalized style.
