Electronic medical records (EMRs) are a digital version of the paper charts in the clinicians office. An EMR contains the medical and treatment history of the patients in one practice. EMRs have advantages over paper records.
A record of information about a persons health. A personal medical history may include information about allergies, illnesses, surgeries, immunizations, and results of physical exams and tests. It may also include information about medicines taken and health habits, such as diet and exercise.
The Public Hospitals Act states that Ontario hospitals must retain records containing personal health information for at least 10 years from the date of the last entry for those over the age of 18 years.
