Right sided ECG leads (V1R-V6R) are positioned in a mirror image fashion from the standard 12-lead precordial leads. Posterior ECG leads (V7-V9) are applied by moving V4-V6 in the posterior positions.

RVMI is considered a contraindication for administration of nitroglycerin to which they are hypersensitive and can develop intractable shock due to dramatic reduction of preload to the left ventricle.

All patients with chest pain should receive a 12-lead ECG early in the patient encounter. When the patient is suffering acute inferior STEMI a right-sided 12-lead ECG can help to identify right ventricular infarction. Be careful with nitroglycerin in the setting of right ventricular infarction.
Right-sided leads The most useful lead is V4R, which is obtained by placing the V4 electrode in the 5th right intercostal space in the mid-clavicular line. ST elevation in V4R has a sensitivity of 88%, specificity of 78% and diagnostic accuracy of 83% in the diagnosis of RV MI.
Right-sided precordial leads are critical to the evaluation of suspected RVMI. Using right-sided precordial leads, ST-segment elevation in lead V4R ≥ 1.0 mm is diagnostic of RVMI. The ECG finding of ST elevation in lead V4R for diagnosis of RVMI has 100% sensitivity, 87% specificity, and 92% predictive accuracy.
When the patient is suffering acute inferior STEMI a right-sided 12-lead ECG can help to identify right ventricular infarction.
Complications of MI include arrhythmic, mechanical, and inflammatory (early pericarditis and post-MI syndrome) sequelae, as well as left ventricular mural thrombus (LVMT) (see the following image).
When a man has a myocardial infarction, there is an average decrease in systolic blood pressure of 10.1 mm Hg between the examination performed before the event and the examination performed an average of 2 years after that examination and an average of 1 year after the infarction (Table I).
