as a nurse working in the ICU I cannot take a report on a pre-made sheet I have to take report on the blank sheet of paper I first start off with the room number and the top part of my page is for any patient demographic information whether or not theyre in isolation so on and so forth allergies all that good stuff then after I run through the systems in sections like this this is what my report sheet looks like when I am finished for whatever reason I just cant do the pre-made sheets I also use colored pens for any changes or any notes that I have to make on this paper how do you guys take report are you using a pre-made sheet or do you use a blend sheet like me
