This should be your full name. It should match the name on your individual tax return. If you have a business name, trade name, DBA name or disregarded entity name, fill it in here. If you do not have a business, you can leave this line blank.
Generally, a W-9 should be updated only if there are changes to the employees personal or business information. This includes changes in marital status, number of dependents, address, and tax identification number (TIN).
You can enter your business, trade or doing business as (DBA) on line 2. If completing the form as a Partnership, multiple-member LLC, C Corporation or S Corporation, enter the name on line 1 as shown on the corresponding tax return as well as the business, trade or DBA name on line 2.
Line one of Form W-9 asks for your name. If you are running a sole proprietorship you would enter YOUR name. To clarify this point, the name on line 1 must match with the name the IRS associates with your TIN. The name on line 1 should never be a disregarded entity a single owner LLC.
