To get a court order giving you permission to change your name, you must file a complaint for name change in the Law Division of the Superior Court. To begin the process of having the court consider your case, you must give the court clerk the necessary forms and a filing fee of $250.
A name change can be processed at any MVC Licensing Center on a walk-in basis. Bring all the required documentation in order to complete this transaction, as well as your 6 points documents. All documents must be original or certified copies and have the required municipal and/or State seals.
Name Change Request Fill out a Change of Name Form. Scan completed form and all supporting documents to your online profile through the Upload License Documents tab. If requesting a duplicate license you will be emailed an invoice for $25 and instructions to pay. Follow up inquiries may be sent to SocialWork@dca.njoag.gov.
You can submit a name or address update by logging into your Nurse Portal account. Please email webmaster@bon.texas.gov if you need the change made to Pearson Vue as well. Please include your name, date of birth, and the last four numbers in your social security number in your email.
In New Jersey, men are allowed to change their name by showing their certified marriage certificate as proof for New Jersey agencies and New Jersey companies.
Please Note: You must submit proof of legal name change in the form of a marriage license, divorce decree, or a court order. Maiden names may not be used as middle names without a court order.
