Collector completes Collector chain of custody in STEP 4 on Copy 1 (signature, printed name, date, time of collection, and name of delivery service) and places the sealed specimen bottle(s) in a leak-proof plastic bag.
Typically, for DOT testing there are 5-parts or copies to a federal CCF. Copy 5 Donor. The format of the CCF is generally specified and standardized by a governing over-site group made up of government and industry representatives such as labs, SAPAA and DATIA.
The only thing that affects a dilute sample is the amount of fluid taken in within a short period of time prior to providing the urine sample. If you are seeing results that show you are getting close to a dilute sample, try to stop drinking any fluids 2-4 hours prior to providing the sample.
A chain of custody form, or CCF, is a document used to keep track of a specimen in the drug testing process. It tracks the specimen from the moment the donor provides the specimen to the collector until the reporting of the specimen analysis from the MRO to the DER.
If you are unable to provide a sufficient amount of urine for a drug test, the collector must begin the shy bladder procedures. This procedure requires that you remain at the collection site.
(3) Ensure that all copies of the CCF are legible and complete. (4) Remove Copy 5 of the CCF and give it to the employee. (5) Place the specimen bottles and Copy 1 of the CCF in the appropriate pouches of the plastic bag. (6) Secure both pouches of the plastic bag.
In order to maintain sample integrity the specimen must be collected in as sterile and dust-free environment as possible. After collection immediately secure the cap on the cup and transfer the sample to at least -20 C cold storage freezer.
- Avoid alcohol, coffee, tea, tobacco and strenuous exercise for 24 hours prior to collection and up to collection time. - Do not consume avocados, bananas, eggplant, pineapples, plums, tomatoes or walnuts for 48 hours before the test.
As you start to urinate, allow a small amount to fall into the toilet bowl. This clears the urethra of contaminants. Then, in the container you are given, catch about 1 to 2 ounces (30 to 60 milliliters) of urine.
