the graph that is taken from the middle third of the patellar tendon is about 10 centimeters long it incorporates a plug of bone from the lower part of the knee cap on one end and a piece of bone from the upper tibia on the other end the defect in the patellar tendon is closed the tendon heals and eventually regains its strength this graft can also be taken from a cadaver this has some advantages but also some risks the rest of the operation is performed with the aid of the arthroscope while viewing the inside of the knee on a television monitor the arthroscope is introduced into the knee and the joint is inspected any associated problems such as torn cartilage is corrected to create a tunnel in the proper location in the tibia a guide pen is passed through the upper tibia it enters the joint at the spot where the ACL normally attaches to the tibia the guide pen is over drilled with a cannulated drill a guide pin is now passed through the tibia tunnel and into the femur it is drilled

The patellar tendon graft (PTG) has always been the gold standard for anterior cruciate ligament (ACL) reconstruction. Yet, most orthopedic surgeons prefer hamstring grafts for younger athletes and cadaver grafts for older patients.
You should soon start seeing improvement in your knee. You may be able to return to most of your regular activities within a few weeks. But it will be several months before you have complete use of your knee. It may take as long as 6 months to a year before your knee is ready for hard physical work or certain sports.
In general, there are three main graft options: a patellar tendon autograft, a hamstring autograft, and an allograft. An autograft comes from the patient's own body, whereas an allograft comes from a cadaver donor.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
Full weight bearing is gradually increased as tolerated by the patient. It typically takes seven to 10 days after the procedure, until the patient is comfortable without the assistance of a crutch.
Symptoms of ACL graft failure Patients may complain of a limp while walking and a feeling of looseness in the knee. There may be knee pain that may increase with activities. Patients may complain of knee stiffness, knee swelling, or instability while activities of daily living.
Generally, those who successfully returned were above-average NFL players before their injury but relatively average after their return. In other words, after an ACL surgery, a Pro Bowl level defensive player regresses to the performance level of an average player and an average player drops below the NFL standard.
Recovery generally takes about nine months. It may take eight to 12 months or more before athletes can return to their sports.
It's important to start walking within a day or two after ACL surgery, but only a little. Walking for a minute or two can help reduce swelling, but you shouldn't walk any more than that. After two weeks, you can start walking around unassisted without crutches for short periods of time.
You should start physical therapy as soon as possible after surgery. How long will I be on crutches? Most likely, you'll use crutches for about a week, then switch to a cane for two weeks.
