The employee may use either a Standard Form 52 or a memorandum to submit a request for a name change. When a memorandum is used, the personnel action may be prepared directly from the information in the employees Official Personnel Folder (or the agencys automated processing system) and on the memorandum.

Change Your Withholding To change your tax withholding you should: Complete a new Form W-4, Employees Withholding Allowance Certificate, and submit it to your employer. Complete a new Form W-4P, Withholding Certificate for Pension or Annuity Payments, and submit it to your payer.
To do this, go to my.calpers.ca.gov and follow the prompts from the Retirement tab to Change Retirement Benefit. You will be able to create and save estimates based on different life event and beneficiary scenarios. You can then elect to make the change online or you can print and mail us your signed election document.

Reciprocity (Changing Retirement Systems) Reciprocity allows you to move from one retirement system to another without losing your benefits. CalPERS reciprocal agreement with other California public retirement systems can allow you to coordinate your benefits between the two systems when you retire.
For federal tax withholding: Submit a new Form W-4 to your employer if you want to change the withholding from your regular pay. Complete Form W-4P to change the amount withheld from pension, annuity, and IRA payments. Then submit it to the organization paying you.
