If the patients name in the medical record or MyChart does not match their current legal name, you may provide any of these documents to request a name change: birth certificate, court order, divorce decree, drivers license, finalized decree of adoption, marriage certificate, Medicaid insurance card, passport or
The Internal Revenue Service (IRS) requires that the name on the Social Security card match the name on W-4 and W-2 forms. Update the employees benefits paperwork. If the change is related to marriage or other qualifying event, the employee may also want to change elections or add or remove a spouse or dependent(s).
You can do so by asking to see their new Social Security Card that shows the name change (it typically takes 14 days for them to receive their new card). If it does reflect a name change, instruct the employee to submit a new Form W-4 with their new legal name, and then update payroll ingly.
Once you identify something you want to change, contact your healthcare provider and request a form for making amendments. Be clear with your request. Upon receiving it, your provider will have 60 days to act on your request. Your provider is not required to make the requested change.
Typically, employers have an HR name change form that an employee must complete, as well as submit the proof of identity and name change forms. This commonly includes the new Social Security card, document of name change (i.e. marriage certificate), and tax forms (i.e. Form W-4, Form I-9).
If an employee has not updated his or her Social Security card, the old name should remain in the employers payroll system until the name is updated. Employees will also need to complete a new W-4.
Notify HR/Payroll Department When it comes to changing your name on employment records, it is important to notify your employer as soon as possible so they can update your employee data, submit updated tax forms, and ensure your benefits and banking information are adjusted.
Inform your employer about your name change so that they can update their payroll and HR records ingly. You may need to provide them with a copy of your court order or marriage certificate. Remember, this is not an exhaustive list, and other important forms and documents may need updating as well.
