DNR protocols typically require the signing of an out-of-hospital DNR order by both the clinician and patient (or the patients surrogate) and the use of a special identifier (eg, a bracelet or brightly colored form) that is worn by or kept near the patient.
Making the Decision If you are near the end of your life or you have an illness that will not improve, you can choose whether you want CPR to be done. If you do want to receive CPR, you do not have to do anything. If you do not want CPR, talk with your doctor about a DNR order.
I understand DNR means that if my heart stops beating or if I stop breathing, no medical procedure to restart breathing or heart functioning will be instituted. I understand this decision will not prevent me from obtaining other emergency medical care by health care professionals prior to my death.
The types of treatments that a patient with a DNR order would not receive include cardiopulmonary resuscitation (chest compressions and mouth-to-mouth breathing, or CPR), electric shocks to the heart, assisted breathing with mechanical devices, or the use of medications intended to start the heart again.
DNR Order Rules Like other complicated medical care in the United States, the application of DNR orders varies from state to state, especially regarding out-of-hospital (ambulance) care. Some states have standardized forms for DNR orders; if the order is not written on that specific form, it cannot be honored.
A DNR order is allowed for any reason. Any adult can request a DNR, including healthy adults. People most likely to use them are those who know in advance that their chance of surviving CPR is very low and/or they dont want to spend their last days on life support.
Regardless of the format or the venue, DNR orders almost always follow the same general rules to be valid: DNR orders must be written by medical professionals rather than verbalized. Written DNR orders must include the patients name. DNR orders must be dated. The DNR order must be signed by the healthcare provider.
Question: What happens if you resuscitate someone with a DNR? If you attempt resuscitation on a person who has a valid DNR order, you open yourself up to lawsuit by the estate (read: family) of the person with the DNR order, especially if the attempt is successful and they are indeed brought back to life.
