Under normal circumstances, when the legal change of name request has already been processed and this application for a legal change of name certificate has been completed correctly, the certificate application is processed by Vital Statistics within 3 business days of its receipt.
To add a father/co-parent using the joint application process, both the mother/person who gave birth and the father/co-parent must jointly apply to have the father/co-parent added to the birth record. Fillable PDF forms do not open properly on some mobile devices and web browsers.
To buy birth certificates online, the birth must have been registered online within the last year. You will require the tracking number from your online registration. You cannot order anyone elses certificates here. Price $40.00 inclusive of service charges.
Cost. There is a $120 government fee for a legal change of name. This fee is for all the name changes included on one application form. Registry agents will also charge a service fee.

Amending a birth certificate If a birth has been registered in Ontario and the parents wish to amend the information recorded on the registration, application forms must be obtained from the Office of the Registrar General, 189 Red River Road, PO Box 4600, Thunder Bay, Ontario P7B 6L8.
