Cervical cancer is cancer of the cervix, the lower part of the uterus that opens into the birth canal. It is one of the most common types of cancer in women worldwide, but also one of the most preventable, thanks to early detection with Pap tests. The cervix has 2 major cell types: flat squamous cells lining the outer part, and column-shaped glandular cells covering the inside of the cervical canal. Both types can become cancerous but squamous cell carcinomas are much more common. Cancer usually starts in the zone where the two cell types meet, known as the transformation zone. Virtually all cervical cancers are caused by human papillomaviruses, or HPVs. There are over a hundred different types of HPV, some of which pose higher risks than others. About 70% of all cases are caused by just two types: HPV-16 and HPV-18. Two proteins produced by HPV, known as E6 and E7, interfere with cell functions that normally prevent excessive cell division. There are five main categories of abnormal Pap smear results within the Bethesda system: Atypical squamous cells of undetermined significance (ASC-US). Squamous intraepithelial lesion (SIL). Atypical squamous cells that may or may not be HSIL (ASC-H). Atypical glandular cells (AGC). Cancer.
The irregular cells identified by Pap test results are classified as low grade or high grade. Low grade changes mean there are mild changes to the cells, and high grade changes indicate moderate to severe changes. These results help your doctor determine if further testing is needed.
Cervical cell classes: (A) normal squamous, (B) normal columnar, and (C) low-grade dysplasia; (D) high-grade dysplasia (HGD) with moderate dysplasia, (E) HGD with severe dysplasia, and (F) carcinoma in situ.
Specimens from some women may also show reactive cellular changes, which is the way cervical cells appear when infection or other inflammation is around.
An abnormal result means that cell changes were found on your cervix. This usually does not mean that you have cervical cancer. Abnormal changes on your cervix are likely caused by HPV. The changes may be minor (low-grade) or serious (high-grade). Most of the time, minor changes go back to normal on their own.
Atypical squamous cells of undetermined significance (ASC-US): This is the most common abnormal Pap test finding. Atypical glandular cells (AGC): Some glandular cells were found that do not look normal.
Class 5 is cancer that has started to grow deeper into the cervix. This stage of cancer can spread to other parts of the body. The new classification system replaces the traditional classes with terms describing the microscopic changes in cervical cells.
LSIL and HSIL are two types of abnormal changes to cervical squamous cells. Atypical Squamous Cells of Undetermined Significance (ASC-US) is the most common abnormal Pap test finding. It means some cells dont look completely normal, but its not clear if the changes are caused by HPV infection.
