Write down all relevant employee information such as their name, role, team or department, shift schedule, and location. List the responsibilities assigned for the day. Describe the assigned tasks and label their status ingly (not started, in progress, completed, and so forth).

Collect the relevant information needed for the report. It should include the patients medical history, current medication, allergies, pain levels and pain management plan, and discharge instructions. Providing these sorts of details about your patient in your end of shift report decreases the risk of an oncoming nurse putting the patient in danger.
A change in status is a change in two (2) or more areas on the Minimum Data Set (MDS). It can be a decline or improvement. It can be physical or mental.
A change in a residents condition may mean that he or she is at risk. Action can be taken only if changes are noticed and reported, the earlier the better. Changes that are not reported can lead to serious outcomes, including medical complications, transfer to a hospital, or even death.
Content of Shift Reports Shift reports may also include favorable and unfavorable or unexpected events and the reasons for these events. For example, a nurses shift report may make note of a patient waking up because of pain.
Whenever a long-term care resident develops an acute illness, such as a urinary tract infection, pneumonia, or a fall in blood sugar, for example, the individual is said to be undergoing a change in condition(COC). Any sudden illness that causes COC can increase the risk of falling.
