Welcome back to my channel, Adventures in America! Today, I'll be discussing how to fill out the CDC Attestation Form. All passengers to the US must fill out this form, with updates including a new version with an expiration date of 6/30/22. Starting April 14, parents of children under two are exempt. The form has two sections for US citizens/ residents and non-US citizens/non-immigrants aged two and older. Find the form on the CDC website or watch how to fill it out in this video!

You can do this 2 ways: through your My Service Canada Account. or. complete the Consent to Communicate Information to an Authorized Person form (ISP-1603) and mail it to us, or drop it off at a Service Canada office.
This helps ensure you have lived here for the required time as a permanent resident. Please remember that for a Verification of Status document, it can take 6 months of processing after you submit an application to IRCC , so make sure you apply ahead of time if you need it for your OAS application.
