POSSIBILITY OF HAVING STIFFNESS IN THE KNEE AND WEAKNESS FOR SURE, AND THE MUSCLES POSSIBLY NOT HEALING AND FAILURE OF THE SYMPTOM RELIEF. IT IS VERY RARE THAT WE GET A PEER GRAFT REJECTION, AS THEY ARE USING CADAVER TISSUE AND THE PATIENT REJECTS IT. THAT IS A VERY RARE OCCURRENCE. IVE NEVER SEEN IT IN MY PRACTICE. I THINK MY BOSS HAS ONLY SEEN IT. ONCE IN HIS 30 PLUS YEARS OF PRACTICING. WHAT YOU CAN HAVE IS DEPENDING ON WHERE THIS COMES IN AND WHERE THE GREAT -- GRAFT IS PLACED, THE PATIENT MAY HAVE AN INTACT LIGAMENT BUT STILL NOT HAVE STABILITY IN THE KNEE AND THAT MIGHT REQUIRE MORE ADVANCED TREATMENTS. ACL grafts are weakest 5 to 12 weeks after surgery. You may feel stronger, but your knee is months from full recovery. Remain cautious.
Unfortunately, ACL graft failures are reported in up to 34.2% in athletes, representing a traumatic and career-threatening event. It can be convenient to understand the various risk factors for ACL failure, in order to properly inform the patients about the expected outcomes and to minimize the chance of poor results.
As many as 200,000 ACL ruptures are reported each year. Reconstruction has a 95% success rate and can be very successful, but it can also lead to challenges such as decreased hamstring strength and loss of proprioception.
Without a properly functioning ACL, the other structures of the knee experience greater strain, which causes further injuries to tissues in the knee. It is reported that 80% of patients with an untreated ACL tear go on to develop damage to the cartilage of the knee. This damage can eventually lead to osteoarthritis.
Symptoms of ACL graft failure Patients may complain of a limp while walking and a feeling of looseness in the knee. There may be knee pain that may increase with activities. Patients may complain of knee stiffness, knee swelling, or instability while activities of daily living.
The surgery may be considered to have failed when objective laxity or patient perception of instability develops in a previously ACL-reconstructed knee, or when postoperative pain and/or stiffness occur in a stable ACL-reconstructed knee. Extensor mechanism dysfunction and infection can also result in failure (6,7).

The most common cause of ACL failure is a technical error in the first surgery. The two most common technical errors are placing the ACL reconstruction graft on the femur too anterior (away from the back wall) or placing the tibial tunnel too posterior (posterior to the anterior horn of the lateral meniscus).
