Go to Insert Header or Footer. Choose from a list of standard headers or footers, go to the list of Header or Footer options, and select the header or footer that you want. Or, create your own header or footer by selecting Edit Header or Edit Footer. When youre done, select Close Header and Footer or press Esc.
Most often, the form Header contains an organizations logo with any common branding elements. The primary benefit is that the form is guaranteed to display the same content at the top of each page of the form.
Add a picture or logo to your form header In Microsoft Forms, open the form you want to edit. Select the form header section. Select Insert Image (picture icon). Search for images in Bing, a OneDrive folder, or your computer or device.
A form or report header appears one time at the top of each form or report. A page footer appears at the bottom of the last page of a form or report.
You can add a header image to Google Forms by clicking the paint palette in the top-right corner. A slide layover sidebar will appear, including the option to Choose image. From here, you can choose one of the many included images, upload an image from your computer, or grab images from your Google Photos account.

To give your survey, quiz, or other type of form some visual flair, apply a theme, add your own background image, or customize the background color. In Microsoft Forms, open the form for which you want to apply a theme. Select Theme to choose a color or background image.
Insert a logo into a form or report Open the form or report in Layout view. How? On the Design tab, in the Header/Footer group, click Logo. The Insert Picture dialog box appears. Browse to the folder where your logo file is stored, and then double-click the file. The logo is added to the form or report header.
200,000 characters is the maximum allowed for total responses per form. For example, if a form contains five text questions, and the respondent has already docHubed the 200,000- character limit by the fourth question, they wont be able to provide a response for the fifth question.
