The COI verifies the existence of an insurance policy and summarizes the key aspects and conditions of the policy. For example, a standard COI lists the policyholders name, the policys effective date, the type of coverage, policy limits, and other important details of the policy.
Certificates of insurance provide a wealth of information, such as benefit coverage and exclusions, benefit limitations, pre-authorization requirements, provider networks, cost-sharing requirements (such as copayments, coinsurance and deductibles), appeals process requirements, patient rights, privacy/confidentiality
COIs contain only the most essential aspects of these contracts with your insurance provider, such as the policyholders name, effective and expiration dates, type of coverage, limits, and carrier.
Call your broker, explain to them what the minimum coverage amount is and that you need proof of insurance. If your policy already meets the requirements, then the broker will contact your carrier to secure the COI.
Certificates should be retained for at least 3 years after the conclusion of the business for which the certificate was obtained.
A certificate of insurance sample should include the name of the company providing evidence of the insurance policy, the identification number of the insurance policy, the type of insurance policy that is included, and the limits of the liability of the insurance policy.
Bold text is a typographic term that refers to a style of text that is thicker and darker than regular text. It is often used to emphasize or highlight specific words, phrases, or headings in a document.
Elements of a COI Company Name and Address: legal name and address of the insured entity. Producer/Agent name and address: the insurance Agent/Broker who issues certificates. Carrier names: name of the company that holds your insurance policy. Policy type: the types of insurance required by the contract.
