The ACL prevents the tibia from sliding too far forward from underneath the femur. It also helps prevent hyperextension of the knee, and resistance to rotational forces about the knee. ACL knee surgery is necessary once the tibia has slid too far or the knee joint has experienced hyperextension.
The ACL also has a major role in resisting internal rotation. The posterior fibers are longest, or most tense, in extension, and are therefore the major resistance to hyperextension and are more susceptible to injury in extension.
Interpretation. Tibial translation or movement of 5 mm or more than movement in the normal limb generally indicates a rupture of the ACL, and more than 2 mm of anterior translation of the affected knee compared to the unaffected knee is considered a positive test indicating ACL injury.
Abstract. The anterior cruciate ligament (ACL) consists of two major fiber bundles, namely the anteromedial and posterolateral bundle. When the knee is extended, the posterolateral bundle (PL) is tight and the anteromedial (AM) bundle is moderately lax.
The anteromedial bundle is tight in flexion (knee bending) and the posterolateral bundle is tight in extension (knee straightening).
The bulk of the fibers of the anterior cruciate ligament (ACL) is taut in maximal extension, while that of the posterior cruciate ligament (PCL) is taut in the intermediate positions and in maximal flexion.
A grade 2 ACL sprain is a partial tear. If you have a grade two sprain, your ligament is damaged and youll likely experiences some instability. Partial, or grade 2, ACL tears are actually the least common of the three types of ACL tears. A grade 3 ACL sprain is a complete tear.
Non-traumatic thickening of the ACL was associated with osteoarthritis and meniscal tears in almost all cases and showed increased signal intensity and ill-defined borders simulating acute ligamentous tears.
The anterior cruciate ligament runs diagonally in the middle of the knee. It prevents the tibia from sliding out in front of the femur and provides rotational stability to the knee. The PCL keeps the shinbone from moving backward too far. It is stronger than the ACL and is injured far less often. Normal knee anatomy.
Between the fibers of the ACL there can be fat or fluid and therefore the ACL is not completely black on PDWI images. Primary signs of ACL rupture are increased signal on MRI, fiber discontinuity, abnormal orientation and undetectable fibers [11].
