The graph from the middle third of the patellar tendon, about 10 centimeters long, includes a bone plug from the knee cap and tibia. The tendon defect is closed, heals, and regains strength. Graft can be from a cadaver, with advantages and risks. Arthroscope aids in knee inspection and associated problems correction. A guide pen creates a tunnel in the tibia, drilling from tibia to femur.

In studies of adults, reports of average ACL length, cross-sectional area, and volume ranged from 26 to 38 mm, 30 to 53 mm2, and 854 to 1,858 mm3, respectively. Reported values were commonly found to vary ing to sex and skeletal maturity as well as measurement technique.
The commonly used allografts for ACL reconstruction are BPTB grafts, HS grafts, tibial is posterior/anterior and tendo achilles grafts.
The mean thickness in the middle third of the ACL was 4.5 mm in the sagittal plane (range: 3.1–7.2 mm) and 4.3 mm in the frontal plane (range: 2.9–6.2 mm). The mean size of the lateral femoral condyle was 62.2 mm (range: 48.1–74.7 mm), while the mean size of the medial femoral condyle was 55.7 mm (range: 43.2–67.4 mm).
Conclusion. In ACL reconstruction, clinical results with autografts are as good as or slightly better than with allografts. Allografts cost more, indicating that autografts are more cost-effective and should usually be first choice.
The three options for torn ACL reconstruction that Dr. Bennett describes in the video are the hamstring autograft, the tibialis allograft, and the patellar tendon graft.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
There are four commonly used grafts for ACL reconstruction. Patellar tendon autograft. The surgeon removes the middle third of your patellar tendon and pieces of bone (bone plugs) from your shin and kneecap. ... Hamstring tendon autograft. ... Quadriceps tendon autograft. ... Allograft.
A grade 2 ACL sprain is a partial tear. If you have a grade two sprain, your ligament is damaged and you'll likely experiences some instability. Partial, or grade 2, ACL tears are actually the least common of the three types of ACL tears. A grade 3 ACL sprain is a complete tear.
The patellar tendon graft (PTG) has always been the gold standard for anterior cruciate ligament (ACL) reconstruction.
The anterior cruciate ligament runs diagonally in the middle of the knee. It prevents the tibia from sliding out in front of the femur and provides rotational stability to the knee. The PCL keeps the shinbone from moving backward too far. It is stronger than the ACL and is injured far less often. Normal knee anatomy.
