The Health and Safety Executive specifies that the following must be reported: Deaths and injuries caused by workplace accidents. Occupational diseases. Carcinogens mutagens and biological agents.
The simple model shown in Figure 1 attempts to illustrate that the causes of any incident can be grouped into five categories - task, material, environment, personnel, and management. When this model is used, possible causes in each category should be investigated. Each category is examined more closely below.
Incident reporting is the process of recording worksite events, including near misses, injuries, and accidents. It entails documenting all the facts related to incidents in the workplace. Incidents are generally accidents or events that cause injuries to workers or damages to property or equipment.
It should include: the names and positions of the people involved. the names of any witnesses. the exact location and/or address of the incident. the exact time and date of the occurrence. a detailed and clear description of what exactly happened. a description of the injuries.
