itamp;#39;s a Dr Weber from USC we just finished allograft KCl reconstruction the patient is a 50 year old avid recreational athlete so we chose allograft over autograph little faster in terms of recovery we did a metal screw in the Achilles allograft on the femoral side again the bone plug on the femoral side so we use the metal screw on the tibial side all soft tissue graft gillies so we used a biocomposite screw we drilled from an anterior medial portal so enter media independent tunnel drilling with a flexible reamer and the case went really smooth she had a little bit of a meniscus tear little contrl defect on the medial side so we clean those things up and nice smooth ACL and she is gonna be ready to get back on the slopes in the back running real soon thanks

In the USA, 68% of MLS physicians prefer bonepatellar tendonbone (BPTB) autograft for ACLR in elite soccer players [6]. Similar trends exist with other sports in the USA as BPTB autograft is the preferred graft of choice for NHL, NFL and NCAA American Football athletes [7-8].
An implant (called the BEAR Implant) is placed between the torn ends of your ACL which enables your own ACL to heal back together. During the procedure, the surgeon adds your own blood to the BEAR Implant and inserts it between the torn ends of your ACL.
Patellar Tendon autograft is likely the best option in younger, higher demand athletes. Hamstring autograft also has high success rate but with less risk of postoperative pain due to a smaller incision and less risk of kneeling pain in the future.
As for ACL graft options, there are allografts (from a donor source) or autografts (harvested from the athlete). The gold standard for professional athletes (or any younger athlete) is an autograft.
Allograft: In this type of surgery, the surgeon replaces your torn ACL with a tendon graft from a donor. Yes, you read that right from a donor! Autograft: In contrast, an autograft involves taking tissue from your own body, usually from your hamstring tendon or patellar tendon, to reconstruct the ACL.
In the young, high-level, or profession athlete, I will almost exclusively use patellar tendon autograft. This is the gold standard for ACL reconstruction with the longest track record of success. Overall, it has the lowest re-rupture rate and lowest likelihood of stretching out over time.
The signs of ACL graft failure can include swelling, pain within the knee, locking within the knee, a mechanical block (which can be due to a bucket-handle tear of the meniscus), lack of full motion, and difficulty with twisting, turning, and pivoting.
1, 2 Still, the most commonly used autografts for ACL reconstruction are the bone-patellar tendon-bone (BPTB) and hamstring tendons (HT). There is some evidence to suggest that BPTB autografts produce more pain than HT autografts both in the immediate postoperative period as well as in the long term.
