ACL injuries are considered sprains and vary in severity. ACL Sprain (Grade 1): The ligament is damaged and stretched but can keep the knee stable. Partial ACL Tear (Grade 2): The ACL stretches and becomes loose. Doctors often refer to this type of ACL injury as a partial tear of the ligament.
ACL injuries are commonly classified in grades of 1, 2 or 3. Grade 1. Grade 1 injuries include ACLs that have suffered mild damage, e.g., the ACL is mildly stretched but still provides adequate stability to the knee joint. Grade 2. Grade 3. Tibial Spine Avulsion ACL Injury.
In general, there are three main graft options: a patellar tendon autograft, a hamstring autograft, and an allograft. An autograft comes from the patients own body, whereas an allograft comes from a cadaver donor. An allograft can come from patellar tendon, hamstring tendon, achilles and other ankle tendons.
Ligaments are 90% type I collagen, but after injury, higher type III collagen volume initially exists [62, 81]. To better withstand loads, the ACL responds to ECM mechanical and chemical environment changes through biomechanical adaptations [62, 111].
As for ACL graft options, there are allografts (from a donor source) or autografts (harvested from the athlete). The gold standard for professional athletes (or any younger athlete) is an autograft.
In the USA, 68% of MLS physicians prefer bonepatellar tendonbone (BPTB) autograft for ACLR in elite soccer players [6]. Similar trends exist with other sports in the USA as BPTB autograft is the preferred graft of choice for NHL, NFL and NCAA American Football athletes [7-8].
Summary. In general it is safe to say that if you are young and active with high physical demands then autograft ACL is the best options. Patellar Tendon autograft is likely the best option in younger, higher demand athletes.
What are the types of ACL tears? Grade 1 (least severe ACL injury). You stretched but didnt quite tear the ACL. Grade 2 (partial tear). You stretched the ACL, making it loose. Grade 3 (most severe ACL injury). A complete or near complete tear the ligament has split into 2 pieces.
