Flame hemorrhages are a subset of retinal hemorrhages occurring within the retinal nerve fiber layer. They are related to pathologies of the superficial retinal capillary plexus,1 including hypertensive retinopathy and retinal venous occlusion.
A jet or flame-shaped hemorrhage is a term sometimes used to describe an intraparenchymal hematoma adjacent to a ruptured arterial vascular malformation, such as aneurysms (most common) or dural arteriovenous fistulae or arteriovenous malformations 1,2.
These glaucomatous disc hemorrhages are flame-shaped and located adjacent to an area of a retinal nerve fiber layer defect. However, when the disc hemorrhage isnt flame-shaped, is broader or not related to a specific RNFL defect, we have to consider other causes.
Three types of hemorrhages are seen at the level of RNFL: flame-shaped hemorrhages, splinter (disc) hemorrhages, and Roth spots. Flame shaped hemorrhages: These hemorrhages are diffuse, found in the posterior pole, and last approximately 6 to 12 weeks.
Superficial or flame-shaped hemorrhages arise from the precapillary arterioles located in the retinal nerve fiber layer. Deep hemorrhages or dot and blot hemorrhages are located in the inner nuclear and outer plexiform layers of the retina.
What is a western blot? The term blotting refers to the transfer of biological samples from a gel to a membrane and their subsequent detection on the surface of the membrane.
Hemorrhages lie deeper in the retina. Usually blood accumulates in the outer plexiform or inner nuclear layers, or more easily seen at peripheral retina where the nerve fiber layer is thin.
