The patellar tendon and hamstring autografts are the most common choices for ACL reconstruction. The patellar tendon runs from the knee cap (patella) to the lower leg bone (tibia). Surgeons have the most experience with this autograft and it is the most widely used.
There are many variations of ACL reconstruction, all of which aim to give the patient the best functioning knee possible. The variability depends on what the graft, the new ligament, is made from. In general, there are three main graft options: a patellar tendon autograft, a hamstring autograft, and an allograft.
Imaging studies such as MRI can help assess the integrity of the reconstructed ACL graft and identify signs of graft failure, such as graft laxity or abnormal signal intensity.
An implant (called the BEAR Implant) is placed between the torn ends of your ACL which enables your own ACL to heal back together. During the procedure, the surgeon adds your own blood to the BEAR Implant and inserts it between the torn ends of your ACL.
The most common cause of an ACL graft failure is a technical error with malposition of the original ACL reconstruction tunnels.
In the USA, 68% of MLS physicians prefer bonepatellar tendonbone (BPTB) autograft for ACLR in elite soccer players [6]. Similar trends exist with other sports in the USA as BPTB autograft is the preferred graft of choice for NHL, NFL and NCAA American Football athletes [7-8].
As for ACL graft options, there are allografts (from a donor source) or autografts (harvested from the athlete). The gold standard for professional athletes (or any younger athlete) is an autograft.
The Lachman and anterior drawer tests are both clinically useful and highly specific, although the anterior drawer is less sensitive than Lachman. However, the lever test should have a more limited role, and findings should be interpreted with caution.
