Common symptoms of a torn ACL include a popping sound at the time of injury, knee pain, swelling and knee instability, especially during weight-bearing activity.
ACL tears are often associated with a popping sound, immediate swelling, and a feeling of instability. MCL tears commonly cause knee pain and tenderness along the inner side of the knee. Meniscus tears may produce a popping sensation, locking or catching of the knee, and difficulty straightening the leg.
Starting with a sagittal view of the lateral aspect of the knee, we move more medial the first thing we see is bone bruising. Bone bruising is usually present with an ACL tear on the anterior aspect of the lateral femoral condyle and the posterior aspect of the lateral tibial plateau.
Instead of seeing normal contour of the dark ACL substance, we start to see more a balled-up appearance. In this view we can also see the injury to the posterior horn of the medial meniscus. Finally, we look at the axial views, although these are not as useful when looking at ACL tears.
ACL tears are also often misdiagnosed as collateral ligament knee strains. With that being said, when a trained musculoskeletal radiologist is reading the case, an ACL tear or other abnormality is rarely missed. This is why a radiologist with subspecialty training is recommended to look at your MRI.
