An unsatisfactory cytology result is reported when there are insufficient cells for the cytopathologist to properly evaluate the specimen. This occurs in approximately 2% of cytologic samples.
There are five main categories of abnormal Pap smear results within the Bethesda system: Atypical squamous cells of undetermined significance (ASC-US). ... Squamous intraepithelial lesion (SIL). ... Atypical squamous cells that may or may not be HSIL (ASC-H). ... Atypical glandular cells (AGC). ... Cancer.
Test results can be inconclusive. This result is sometimes called ASC-US, which means atypical squamous cells of undetermined significance. The cells didn't look quite like normal cells, but they couldn't really be classified as abnormal. In some cases, a poor sample can lead to inconclusive results.
Inflammation on a Pap smear can be found in a patient of any age and may be attributable to a benign infection—such as Candida vaginitis—that need be treated only if the patient is symptomatic. Sexually transmitted infections can also cause an inflammatory reaction on the cervix and should be treated ingly.
Avoid intercourse, douching, or using any vaginal medicines or spermicidal foams, creams or jellies for two days before having a Pap smear, as these may wash away or obscure abnormal cells. Try not to schedule a Pap smear during your menstrual period. It's best to avoid this time of your cycle, if possible.
Not use vaginal creams, foams, gels, douches or tampons 48 hours (two days) before your Pap smear. These products could cause an abnormal result. Not have sex 48 hours (two days) before your Pap smear. Sex can irritate the skin of the cervix or cause discharge (fluids) that can lead to an abnormal result.
Conclusion: Most patients with inflammatory smear results are managed by their general practitioner without reference to specialist services. Many patients are not investigated for infection but treatment often includes medication which covers the most likely or potentially serious genital pathogens.
Mild inflammation was defined as less than 30 inflammatory cells/high-power field; moderate inflammation was defined as 30 to 100 inflammatory cells/high-power field; and severe inflammation was defined as more than 100 inflammatory cells/high-power field.
In most cases, an abnormal Pap test is a result of: A human papillomavirus (HPV) infection. A sexually transmitted infection (STI or STD), such as herpes or trichomoniasis. A bacterial or yeast infection.
Cause. Most abnormal Pap tests are caused by HPV infections. Other types of infection—such as those caused by bacteria, yeast, or protozoa (Trichomonas)—sometimes lead to minor changes on a Pap test called atypical squamous cells.
