In this lecture we re gonna cover the pharmacology of drugs used in treatment of heart failure, so let s get right into it. Heart failure is simply defined as a chronic, progressive disorder in which the heart muscle is unable to pump enough blood to meet the bodys needs. In a normal heart, the upper chambers called the atria and the lower chambers called the ventricles squeeze and relax in turn to move blood through the body. Now blood flows through the heart and lungs in four major steps. First, the oxygen-poor blood that has already circulated through the body is received by the right atrium, which in turn pumps it over to the right ventricle. Secondly, the right ventricle pumps the blood through the pulmonary artery into the lungs, where it picks up oxygen. Thirdly, the pulmonary vein empties oxygen-rich blood, from the lungs into the left atrium, which in turn pumps it to the left ventricle. And finally the left ventricle pumps oxygenated blood through the aorta to the rest of t

Most commonly, it begins in the hearts primary pumping chamber - the left ventricle. Each specific type of CHF is accompanied by its own distinct characteristics: Right-sided Congestive Heart Failure - Right-sided CHF develops when the right ventricle struggles to deliver blood to the lungs.
When the left side of the heart is failing, it cant handle the blood its getting from the lungs. Pressure then builds up in the veins of the lungs, causing fluid to leak into the lung tissues. This may be referred to as congestive heart failure.

has been reported to have detrimental effects following myocardial infarction. The drug may increase myocardial oxygen consumption35 by augmenting contractility and inducing peripheral or coronary vasoconstriction. It may also provoke ventricular arrhythmias and increase infarct size.
These include an increase in contractile function, increase in heart rate, cardiac myocyte hypertrophy, and volume expansion/increased end-diastolic volume (via -adrenergic signaling of nonosmotic vasopressin release).
These may include: Angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers (ARBs) and angiotensin-receptor neprilysin inhibitors (ARNIs) to lower blood pressure. If (pronounced I-F) channel blocker and beta blockers to reduce heart rate.
As the right heart failure worsens, the upper legs swell and eventually the abdomen collects fluid (ascites). Weight gain accompanies fluid retention and is a reliable measure of how much fluid is being retained.
Generally, heart failure begins with the left side, specifically the left ventricle your hearts main pumping chamber. Fluid may back up in the lungs, causing shortness of breath. Fluid may back up into the abdomen, legs and feet, causing swelling.
