Specific bony landmarks assessed for the ACL tibial attachment include the lateral and medial tibial eminences, the medial and lateral tibial pla- teau articular cartilage borders, the ACL ridge, the ACL tubercle, the anterolateral fossa, and the ret- roeminence ridge [1].
ACL surgery involves reconstructing the ligament. Surgeons use screws to attach the reconstructed ligament to the femur (thigh bone) and the tibia (shin bone). A person may experience pain after surgery due to the placement of these screws. Pain may also be a sign of ACL surgery failure.
3-5 The ideal placement of the tibial tunnel should be 9 mm posterior to the intermeniscal ligament, in line with the anterior horn of the lateral meniscus, and approximately 7 to 10 mm anterior to the posterior cruciate ligament insertion.
A torn ACL cant heal on its own, but its possible to live with it (especially if you have a low-grade tear). But if youre an athlete or want to return to physical activity, youll need surgery to repair your ACL. Most people choose to have an ACL tear surgically repaired.
A bad sprain can involve a small tear, but in most cases, it is only when the ligament has been stretched too far. ACL injuries are graded on a scale of one to three. Your doctor will determine the grade of injury you have following a physical examination and by using imaging tests such as X-rays or an MRI.
It is located on the anterior aspect of the tibial plateau on the intercondylar area. 3-Dimensional (3D) computed tomography (CT) scan is now commonly used to assess appropriate tunnel position, and this frequent use leads to technical improvement for individual surgeons.
Ideally, the tunnel placement should be centered one-quarter of the total facet length anterior to the posterior cortex of the tibia (7).
Symptoms of an ACL Injury? Pain on the outside and back of the knee. Knee swelling within the first few hours of the injury. Limited knee movement because of swelling and/or pain. Loss of full range of motion. Discomfort while walking.
