An affidavit is a legal document used in court and other authorized contexts, made by one person in the presence of an authorized affidavit taker. Different courts and tribunals may have specific format requirements, so it's advised to check their websites beforehand. You can complete most of the affidavit form before your appointment, but do not sign it until you are with the affidavit taker. A list of authorized affidavit takers is available at justice.vic.gov.au. During the signing, you must initial any changes and sign each page of the affidavit, which the affidavit taker must also sign after any alterations and on each page. If the affidavit references a document, the taker must attach a certificate identifying it.
